This can include patients who have had severe trauma, who are heavily sedated, and who are intubated (not breathing on their own and have a tube down their throat into their lungs to breath for them). These are people that are either completely out of it or close, whether or not medications are involved. This is not a new term. My 62 year old mother who is soon to be a retired nurse has used the term and I, as a medical worker have used it too.
